Parent's instruction for medical care in case of emergency

PLEASE ATTACH THE FOLLOWING:

1. A photocopy of the applicant's birth certificate

2. A photocopy of Immunization card

3. Two (2) Passport Photograph

4. Print out of the filled admission form

5. Signed school policy sheet

6. Transfer certificate and previous term's report sheet (for transfer students only)

NOTE:

1. Pupils will only be considered for admission into Creative Learning International School when the Admission Forms are completed and returned with all relevant documents.

2. By signing this form, you give Creative Learning International School permission to contact your child's pervious school for any relevant information that could be pertinent to the child's overall assessment.

3. Admission is subject to availablity of space and pupil satisfying the admission requirement.

4. School fees are to be paid in advance of every school year.

5. A non-refundable application/registration fee of (Naira) 10,000 (Ten Thousand Naira Only) is required for purchace of school Admission form.

6. A holding fees of (Naira) 100,000 (One Hundred Thousand Naira only) for primary pupils is required to secure a place. Note that the holding fee represents part payment of the school fees.

7. Once a pupil has commenced a term, fees for the term cannot be refunded.

8. All fees should be paid into any of the School's bank accounts. Copies of the deposit slip showing payment made should be forwarded to the school accountant via email - [email protected].

9. All payments are to be made in the bank (Application/Registration payment can be made on our website).

10. For every new enrolment, payment has to be made in full upon resumption.
